Back to School Forms

Please ensure that your student's Back to School forms are complete.

To complete the forms login to Skyward Family Access.

Select Online Forms to view the forms that need to be completed for each of your children in the district.  

Please take a moment to complete the 26-27 Back to School Forms and to sign the Device and Network Agreement form for your student ASAP.

This agreement helps ensure students have access to the technology and network resources they need for learning throughout the school year.

To prepare for your student to use a device:

Log into Skyward Family Access (If you need Skyward login assistance, contact your child’s school office).

Complete the Technology Use Agreement. This Agreement is located under Online Forms on the left side in Skyward Family Access.

Within this form, you’ll read the Technology Use Agreement.

Check the Box Agreeing to the Terms of Use.

Provide an electronic signature.
